Forth maintains an ever-growing roster of clinicians at the top of their field, ready to become a practice owner. At the same time, Forth looks for practices with excellent fundamentals, but with an owner/clinician nearing retirement. Once a match is made, Forth acquires the practice and offers favorable buy-in financing for our partner clinician. Forth and the partner doctor become long-term partners in the practice.

An experienced dental investor & entrepreneur, Cole founded Forth Equity in 2023, an investment firm focused on partnering with entrepreneurs & operators in fragmented, stable industries. Prior to founding Forth Equity, Cole co-founded Carolina Elite Dental, a successful Dentist Focused DSO, as well as worked alongside industry leading private equity investors in the Dental Space. Forth Dental is born from the limitations of traditional DSO models, a much more personalized and dentist-centric approach. Alongside Dr. Offutt, Cole remains committed to addressing the industry’s overlooked segments, fostering long-term relationships, and prioritizing clinical excellence.
Outside of work Cole has a growing young family and loves to stay active – an avid runner, tennis player, and golfer.
With over 25 years of experience in the dental industry, Deena brings a wealth of knowledge & expertise to Forth Dental. As the Operations Manager, Deena ensures that our offices run smoothly & efficiently, allowing our dental professionals to focus on providing top-notch care to our patients.
Her passion for patient care and operational excellence is evident in everything she does, from streamlining administrative processes to enhancing patient experiences.
In her role, she oversees all aspects of the practice’s operations including patient relations, financial management, and compliance with healthcare regulations.
